Contact the Office with a Request

You may use this composer to send an e-mail request for an appointment, referral, prescription renewal, or any other message. This service is provided as a convenience and is neither confidential nor secure. No guarantee or warrant is made regarding the timliness of receipt or response. No diagnosis, treatment or advice will be given without an appropriate interview and examination.

Appointments
When requesting an appointment please include your full name, address, work and home phone numbers, and health insurance information. Indicate the date and time you prefer and basic information about why you are visiting. In this way we may reserve the appropriate amount of time. You will be contacted as soon as this message is retreived.

Referrals
Many health insurance carriers require prior authorization before considering payment for services rendered outside our office. They refer to this an a "referral." If you suspect you have a medical problem which may require specialized care or consultation with a specialist, an examination in our office will help us determine with you the best course of action. Without our evaluation you may make an incorrect assumption about the nature of your problem and the appropriate source of care. As a general policy, referrals will require an appointment in our office.

Requests for referrals will only be accepted by e-mail for continuation of ongoing care after a previously referred initial evaluation from a specialist, or for routine tests like mammography, bone density, etc.

Prescription Refills
If you need a prescription refill it's probably time to return to the office to update your status. We generally limit the number of refills for ongoing medications as a way to remind you when it's time to return. When you see your supply running low, please call for an appointment before you run out. In an emergency situation we will consider prescribing a small supply until you are able to visit.

Some pharmacy plans have no limit on the number of refills you may have. However, that ignores the importance of ongoing medical supervision to determine the appropriateness of chronic therapy. Things can change which may not be suspected, and a new approach may be needed.
